How to review your PCI compliance form

Verify your PCI compliance form

When you download your PCI compliance form from Titus, we prefill it with your business information from your account setup. Before you submit it, take a few minutes to make sure everything is accurate.

What to check before submitting

Legal business details
  • Legal business name: Does it match your EIN documentation or operating agreement exactly? This is the most common source of errors.

  • Business address: Is this your current mailing address where you receive official documents? If you've moved offices or relocated, update it now.

  • EIN or Tax ID: Verify this matches what you provided when you signed up for card payments.

  • Contact information: Make sure the name, phone number, and email are current for whoever will be signing the form.



PCI compliance specifics
  • Data storage questions: The form should confirm that you do NOT store cardholder data yourself. Titus and Finix handle all the secure processing.

     

  • Security requirements: You should not see any "Yes" answers to questions about implementing firewalls, encryption systems, or security protocols. If you do, that's a red flag — contact support before submitting.

     

  • Card processing: The form should state clearly that cards are processed through a certified payment processor (Finix), not through your own systems.


Red flags to watch for


**Don't submit if you see:**

  • Blank fields or placeholder text that wasn't filled in

     

  • Your old business name, address, or phone number

     

  • "Yes" answers to questions about you storing or handling card data

  • Questions you don't understand or can't answer


If anything looks wrong, stop and reach out to Titus support before submitting. We'd rather help you correct it than have you submit inaccurate information.

 

If your business has changed

If any of these have changed since you set up your account, contact us before submitting the form:

  • Legal business name

  • Business address

  • EIN or Tax ID

  • Business structure (became an LLC, S-Corp election, etc.)

  • Number of locations or how you operate

Changes like these usually require a new application rather than a simple edit to your existing form.

 

Quick checklist

Before you hit submit, check off these three things:

  • Legal name is exactly right

  • Address is current

  • Contact info matches who's signing this

That's it. If all three check out, you're good to submit.
